013 Bad Posture
Situation
A man had a syncope, large postural drop at scene. But it was just a
vasovagal syncope.
Background
A 55 year old man had classic symptoms a vasovagal syncope [the 3 Ps].
Paramedics recorded a 40 point drop between lying and standing blood
pressure at home. In ED he was fully recovered, no concerning features
in the history or exam. There was no longer any postural
hypotension.
Assessment
He had a vasovagal syncope which, during the acute phase, can manifest
as postural hypotension. Once recovered there was no longer any postural
hypotension and he was discharged.
Recommendation
Know that postural hypotension can occur during a vasovagal episode and
if recovered, there is no need to investigate for or admit to
hospital.
