025 3+ Misdirection

Situation

Patient had microscopic haematuria: it was diverticulitis.

Background

A 40 year old lady, otherwise well presented with abdominal pain and D&V. Inflammatory markers very high. Left sided abdominal and renal angle tenderness. Urinary dip: only 3+ blood. CT showed acute diverticulitis with perforation.

Assessment

The diverticulitis was irritating the ureter and causing haematuria, there was no renal stone or pyelonephritis.

Recommendation

Microscopic haematuria can be caused by surgical problems such as diverticulitis, colitis and appendicitis.

Before diagnosing a UTI or renal stone just consider a non-urological cause too.
