028 Bad Food Guide
Situation
Patient had supra pubic pain and vomiting: it was an MI.
Background
An elderly lady had eaten “something rotten” and then developed lower
abdominal pain and vomiting through the night. She had an NSTEMI 6 weeks
earlier. Obs, ecg and exam were unremarkable. Troponin added on: was
22000, treated as another MI.
Assessment
Despite no chest pain she was having a significant MI. [Not
gastroenteritis] We know that some groups such as elderly women and
diabetics can suffer an MI with little or no actual chest pain.
Recommendation
In these patient groups if you can’t find a cause of them feeling
unwell, consider ACS even without chest pain.
In this case she’d had a recent MI which raised suspicions
further.
