050 Trifasicular Trouble
Situation
An elderly lady had trifascicular block on ECG.
Background
An elderly lady presented with 2 weeks of chest pain, no collapse or
palpitations. Her ECG showed a trifascicular block. She was admitted and
later Cardiology reduced the dose of her Diltiezam.
Assessment
It doesn’t appear that her chest pain was related to her abnormal ECG.
In this case the cause of her trifascicular block was thought to be the
Diltiezam.
Recommendation
Know that trifascicular blocks can be complete or incomplete [as in this
case], it looks like like this:

Know that, trifascicular blocks. if newly diagnosed, usually need further evaluation whether they are symptomatic or not.
They can go on to become complete heart blocks.
