065 Ruptured Serenity

Situation

Abdominal pain was renal collecting system rupture.

Background

A 75 year old woman had sudden severe left sided abdominal pain. She was tender on the left and in the renal angle. Old imaging showed severe hydronephrosis from ureteric stricture.

Assessment

WCC 15, no reported haematuria. CT abdomen showed renal collecting system rupture, extravasation of urine into the abdomen.

The patient had urgent nephrostomy.

Recommendation

Be aware of this complication of hydronephrosis.

Reviewing past scans is quick and helpful.
