069 Not Tender

Situation

Fournier’s gangrene wasn’t tender to touch.

Background

A man presented with sepsis. His scrotum was swollen, erythematous with black necrotic areas. The scrotum was not tender to touch or move.

Assessment

The Fournier’s gangrene had thrombosed the blood vessels and destroyed the nerve endings, reducing the sensation of tenderness.

Recommendation

Know that pain and tenderness is a hallmark of Fournier’s gangrene. However as the disease progresses these may be absent.
