071 Avulsion Revulsion

Situation

Adolescent had ischial tuberosity avulsion fracture.

Background

A 15 year old boy back heeled a football, felt a sudden pop, crunch around the gluteal fold of his leg. Presented to ED with only slight limp and full ROM in hip. X-ray pelvis showed an ischial tuberosity avulsion fracture. Discharged with non weight bearing crutches, fracture clinic and physio.

Assessment

A sudden forceful contraction of his hamstrings caused an avulsion fracture at the site of attachment onto the pelvis. This was not a simple hamstring strain.

Recommendation

Know that in teenagers avulsion fractures of pelvic apophysis sites can occur, at these sites:
