072 Torsional Contortion
Situation
Amylase was 700 but diagnosis was ovarian torsion.
Background
A woman presented with pelvic pain with amylase 700, CT showed ovarian
torsion not pancreatitis.
Assessment
The rise in amylase was caused by a non-pancreatic abdominal pathology,
in this case ovarian torsion.
Recommendation
Remember: to diagnose acute pancreatitis you need 2 of the the following
3:
Typical history.
Elevated serum amylase or lipase (>3 ULN).
Imaging (CT, MRI or ultrasound) consistent with acute pancreatitis.
