075 A Rashional Decision
Situation
Shingles treated with both aciclovir and steroids.
Background
A middle aged man presented with a painful shingles rash on his chest.
He was given aciclovir and prednisolone [with PPI] and discharged.
Assessment
He was in a lot of pain, meeting the indication for prednisolone in
addition to aciclovir.
Recommendation
“Consider oral corticosteroids [in the first 2 weeks following rash
onset] in immunocompetent adults with localized shingles if pain is
severe, but only in combination with antiviral treatment.
Use clinical judgment, taking into account the risks and benefits of corticosteroid therapy for each person.”
